New Patient Enrollment

Before enrolling, please make sure you have:
  • Completed the New Patient Intake Form
  • Communicated with Jen Dobie, our clinical intake specialist, and were instructed to complete enrollment forms after setting an appointment

Getting to our offices

  • Our main clinic is located at 1235 SE Morrison Suite 100. We are located across the street from Zells Cafe in the MORRISON PLAZA building. We are the only office suite on the ground floor.

  • Please feel free to park in our parking lot off Morrison Street in the reserved spaces that face the construction site.

  • We offer bike parking just outside our front doors.

  • Our office building is ADA compliant and accessible to people with disabilities.

  • Dr. Randhava's office is located at 3810 SE Division Street Suite C. This is just around the corner from Little Big Burger. There is free street parking. The office is ADA compliant and accessible to people with disabilities. Bike parking is available.

Office etiquette

  • Kindly arrive on time

  • At the main clinic, please check in using the iPad on the table. Your therapist will be notified of your arrival and will soon greet you in the lobby. Please do not wander the office.

  • At Dr. Randhava's office, you will be received in the lobby at the time of your appointment.

  • Make yourself comfortable. Please feel free to hop on our wifi (Password: Restore2) and grab a beverage. We offer Ristretto Roasters coffee, tea and water.

  • Please use headphones and mute your cell phone to avoid disturbing sessions in progress.

  • Toward the back of the office, we offer a waiting space for children up to 11 years old that is stocked with toys, books and a teepee.

Cancellation Policy

  • If you need to cancel or reschedule your appointment, please do so by calling us at 503-376-7114 and leaving a voicemail for your clinician.

  • If you cancel, you must give us 24 hours notice to avoid being charged your full session fee.

Billing Policy

  • Restore Therapy will bill your insurance if we take your plan and receive authorization. Currently in-network plans include Kaiser, Pacificsource, Health Share and FamilyCare.  

  • If we are not in-network with your insurance company, your fee is due at the time of the session. We can charge your account on file or take cash or check. As a courtesy, we can provide an invoice that you can submit to your insurance for your own reimbursement.

  • We require all new patients to have a credit card on file to hold your appointment. If we do not yet have your card on file you may enter it by clicking the "Provide your payment information" button above.
